This is a relationship-first role. You will be trusted to represent our mission with professionalism, warmth, and a deep understanding of the home health landscape.
Key Responsibilities
• Build and nurture referral relationships with physicians, hospital discharge planners, skilled nursing facilities, assisted living communities, and community organizations
• Educate healthcare providers on home health eligibility, Medicare benefits, and the full scope of Prime Home Health's clinical services
• Serve as a trusted advocate for patients and families navigating post-acute and home-based care options
• Grow the referral network through consistent, strategic outreach and diligent follow-up
• Track outreach activities, referral sources, and relationship-building efforts accurately in our EHR (WellSky)
• Collaborate closely with our clinical and intake teams to ensure a seamless, patient-centered admission experience from referral through start of care
• Support the clinical and admissions team to ensure patients receive timely, high-quality care from first contact through start of care

Base Salary: $50,000 – $100,000+ annually, based on experience and existing referral network
Performance Bonus: Monthly performance bonus based on individual contributions
Schedule: Monday – Friday, with occasional flexibility for community events

• Solid understanding of home health regulations, Medicare eligibility criteria, and PDGM
• Genuine passion for home-based care and patient advocacy
• Excellent interpersonal and communication skills — you are known for being warm, professional, and trustworthy
• Self-directed and organized — you manage your territory like a business
• Comfortable with technology including EHR platforms (WellSky experience is a plus), MS Office, and communication tools
• Valid driver's license and reliable transportation

• Competitive base salary ($50,000 – $100,000+) with monthly bonus incentives
• Medical, dental, and vision insurance
• Paid time off, sick leave, and paid holidays
• Mileage and phone reimbursement
• A supportive, mission-driven team that genuinely cares about each other and the patients we serve
